Create App Questionnaires
Each questionnaire consists of pages and there are elements of different kinds.
When creating, the page with its elements is displayed on the left and a column with configuration and input fields on the right. Each page and each element has a header with the wrench as the edit icon. By clicking on the edit symbol, the input fields for this element (or page) are displayed on the right and you can configure it.
In the “Questionnaire catalog” () you create a new questionnaire by clicking on the plus symbol .
Now the new questionnaire appears. To give it an individual name, click on the edit icon next to “New questionnaire”. The “Title” field then appears on the right. Here you can enter the desired name and then save it by clicking on “Apply”.
To add a page to the questionnaire, click on “Add another page”. A box appears on the left, you can use the edit symbol to specify the title in the right column.
Within the box you can now add the desired elements. Please click on “Add another element”. A ‘box in the box’ appears, in the header line the number of the element and the wrench for editing are displayed. In the right column, in the Type field, you can specify which element you want and, depending on the element, the content. For example, you can select a heading, a multiple selection, or a signature field.
To add another element, click on “Add another element” at the desired position – either above or below the existing element.
Pages and elements can be moved by drag&drop (drag and drop), the numbering adapts.
You can delete an element or a page by clicking on the trash can icon.
selection Here you can enter a simple question with a given number of possible answers. However, only one answer can be selected.
The photo element allows you to add pictures to your questionnaire at the desired position.

You can ask a question that can only be answered with Yes or No.
choice A question with predefined answer options, where multiple selection is possible.
Here an evaluation can be inserted, which is represented for example from 1 to 10. Setting is possible via a slider to the right and left
The “Text” element allows you to set a questions which is to be answered by the customer with a sentence.
In this element, the customer can respond to your question in more detail than in the “Text” element.
This element allows you to request a digital signature from the customer. If this item is filled in in the appointment, you can no longer edit the previous questions.
This element is similar to the scale, you can also predefine in which number range the result should be. Selection takes place via “+” and “-“.
You can use this element to add headings to pages or sub-items.
If you are satisfied with your settings, click on the Save button in the upper left corner.
You can then use the questionnaire in the onOffice app.
